SAN DIEGO and WASHINGTON, Dec. 18, 2015 /PRNewswire-USNewswire/ -- Today, the West Health Institute commended the Senate Finance Committee Chronic Care working group for the release of their Chronic Care Options Paper which outlines policies under consideration that aim to improve chronic disease management for Medicare beneficiaries.
