ATLANTA and TAMPA, Fla., June 9, 2016 /PRNewswire/ -- Today there are more than 53,000 clinicians and 11,000 practices across all 50 states with Patient-Centered Medical Home (PCMH) recognition. Understanding that PCMH has become an endorsed model of primary care, WellCare of Georgia, a subsidiary of WellCare Health Plans, Inc. (NYSE: WCG), is integrating new supports, tools and processes into its Enhanced PCMH Initiative to assist physician groups seeking National Committee for Quality Assurance (NCQA) recognition.
WellCare established its Enhanced PCMH Initiative to assist primary care practices in Georgia in becoming recognized as a PCMH to increase the number of practices committed to fostering strong physician-patient relationships and encouraging proactive care coordination while improving the quality of care provided for its members.
PCMHs foster ongoing partnerships between patients and their personal clinicians. Each patient's care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research conducted by NCQA shows that PCMHs can lead to higher quality and lower costs and can improve patient and provider reported experiences of care.

In addition to the BizMed platform, WellCare of Georgia also offers consulting assistance during the recognition process as part of its Enhanced PCMH Initiative. This assistance includes reviewing documentation, monitoring progress, and educating and interpreting the PCMH requirements.
In addition, WellCare of Georgia and NCQA have partnered to provide a discount to smaller health care practices in Georgia that seek recognition as an NCQA PCMH.
