CINCINNATI, Oct. 22, 2012 /PRNewswire/ -- PatientPoint®, the leader and innovator in point-of-care patient and physician engagement solutions, today announced that the University Physicians Group (UPG) will implement the PatientPoint Care Coordination platform and its electronic Check-In/Check Out process, which will help enhance patient engagement, increase workflow efficiencies and improve clinical outcomes. UPG will use the Care Coordination platform in conjunction with the practice's existing PatientPoint Waiting Room Networks digital health education content to provide patients with valuable information and comprehensive care before, during and after their visit.

For more than 21 years, UPG, a multispecialty medical group, has been serving Staten Island and southern Brooklyn. "We strive to provide quality care that drives successful outcomes and keeps our patients healthy," said Dr. Jeffrey Hyman, chief medical officer of UPG. "Innovative technologies, like the PatientPoint Care Coordination platform, enable us to bring patient care to the next level, especially as we work towards NCQA certification in the diabetic program and enter into the area of patient-centered medical homes. With these new tools, we will be able to better identify gaps in care, more effectively work with patients to manage their conditions and adhere to medications, and ultimately improve patient outcomes."
The PatientPoint Care Coordination platform automates and streamlines the clinical cycle by aggregating data from health plans, practice management systems, EMRs and pharmacy partners, allowing UPG medical practices to easily identify gaps in care and connect with patients to close the identified gaps.
As part of the implementation, UPG will deploy electronic Check-In/Check-Out iPads at the point of care to digitize and streamline the patient intake process, while alleviating administrative burdens often encountered in the front office, back office and during the clinical visit. On the PatientPoint iPad, UPG doctors will capture vital patient self-reported information. The PatientPoint iPad also provides patients with customized intervention programs, health screenings and assessments necessary for better patient treatment and outcomes. At check-out and post-care, patients are offered continuing adherence and compliance programs, and enrollment options for disease management and wellness programs.
"As the healthcare industry increasingly transitions to accountable care organizations and patient-centered medical home models, more providers are realizing the value of tools that allow them to effectively engage patients in the care process and manage patients' health, not only at the individual level but also at the population level," said Raj Toleti, president of PatientPoint Coordinated Care Solutions. "Leading organizations like UPG are at the forefront of reform, paving the way for others and illustrating what it takes to positively impact the way care is delivered to patients today."
