Within 24 hours of patients checking into the hospital, Pieces™ tells care providers who is most at risk for readmission. Pieces™ then helps providers focus specific services and care on high-risk patients to guide them through their hospital stay and beyond discharge to keep them healthy and out of the hospital.

PCCI and University Health System will work together to optimize existing care plans for high risk patients in order to get the best possible results from Pieces™ for the Health Center’s patients.
