UHS-Pruitt will deploy the Caradigm Intelligence Platform (CIP) to unify clinical, operational, and financial data stored in diverse systems and make it available in near-real time within workflows. The ability to rapidly access consolidated data will provide greater insight into patients and populations, enable clinical and operational surveillance, and improve communication among extended care team members and families. Once consolidated in a central location, the data can serve as the foundation for building additional tools and applications designed to improve patient care and organizational performance.
"Innovative software is critical to our efforts to continuously improve and advance the quality of care that defines UHS-Pruitt," said Neil Pruitt, Chairman and CEO of UHS-Pruitt Corporation. "Caradigm solutions will allow us to use the data stored throughout our organization to better manage care according to patient need. The software also will help us establish and maintain best practices in clinical care and operations across our business units."
UHS-Pruitt will use the Caradigm Intelligence Platform (CIP) to give care teams a centralized, longitudinal patient record, helping them to more easily coordinate care as patients transition to new healthcare settings, monitor patient progress continuously, and share reports with families. The longitudinal patient record also will allow administrators and unit directors to identify and analyze trends across patient populations to help manage risk and improve care processes.
The organization also will use CIP to help reduce avoidable patient re-hospitalizations through ongoing surveillance to identify and track at-risk patients and compliance with actions needed for that population. This application will support UHS-Pruitt's UniGuard program that focuses on decreasing Medicare patient hospitalizations.
"With the shift toward serving higher-acuity patients, post-acute healthcare organizations face challenges similar to those of acute-care hospitals, including managing care transitions, reducing avoidable re-hospitalizations, and improving care quality while reducing costs," said Michael Simpson, CEO of Caradigm. "We're excited to partner with UHS-Pruitt to demonstrate how a leader in post-acute care can use data strategically to make a positive impact in all of these areas and improve the lives of patients and their families."
