TWN and ACP partner on new patient education 'prescription'
NEW YORK, Jan. 20, 2016 /PRNewswire/ -- The Wellness Network (TWN), and the American College of Physicians (ACP) are partnering to deliver new patient education programming that will be available via The Wellness Network's Patient Channel, an in-hospital TV network and online portal. The patients and caregivers at Patient Channel hospitals will have immediate access to a library of education resources developed by the ACP to help manage chronic conditions. ACP member physicians will have access to the Patient Channel library of disease and medication management, wellness and patient safety videos that can be viewed by patients during an office visit or at home.
The new collaboration offers enhanced engagement between patients and physicians across the continuum of care, using the power of video as an educational and motivational tool. From outpatient visits to in-hospital stays, education can be "prescribed" by physicians and delivered at a time when patients are most in need of key information and highly motivated to take action to improve their health.

The partnership with TWN supports ACP's mission to enhance the quality and effectiveness of health care. Educational content from ACP features an ACP member physician interacting with patients -- focusing on key, how-to steps for managing atrial fibrillation, COPD, heart failure, diabetes, and asthma.
