Therap's LTSS Software Enhances Person Centered Data Homepage

NEW YORK, March 29, 2016 /PRNewswire/ -- Therap Services introduced an enhanced user interface to the Individual Home Page.  Therap's LTSS software's complete suite of human service documentation includes service authorizations, case management, health tracking, billing, daily progress notes and medication administration records. Users include state disabilities agencies and private provider agencies.

Therap access is based on person-centered rights and privileges. People should view and access only the data they need and are authorized to see. An individual or family member only needs access to one person - the Individual Home Page is a great way of viewing that data. 

In addition to families and self advocates using this dashboard, the personalized homepage is for nurses, case managers, QIDPs and other support professionals.   Agency-wide reports are accessed throughout Therap while relevant information pertaining to a specific individual is viewable on both their Home Page and elsewhere.

The Individual Home Page displays information from several Therap Individual Support modules, including; individualized plans, care plans, behavior plans, case management notes, progress notes, service and outcomes delivery data, attendance tracking and scanned documents. Case managers may check the case status of individuals they have access to, including program enrollment and discharge dates. The Individual Home Page includes a Profile tab that displays the identification data, contacts, health information and active allergies of an individual receiving Long-Term Services and Supports.

The Individual Home Page allows for case managers to reference goals and individual outcomes statuses. They may review team plan minutes and create ISP Agendas to drive meetings based on need and areas identified through the individual's Personal Focus Worksheet. Interconnected modules ensure that service data collected in Therap's developmental disabilities software generates reports and drives the progress of outcomes.

The enhanced Individual Home Page provides an overview of relevant health information pertaining to an individual, including demographic and contact information, active medications, allergies, diagnoses, guidelines and order details. Reminders of upcoming appointments and pending immunizations alert users of important scheduled health engagements. Users are notified of recent progress notes, communication log entries, and event/incident reports for that individual. The updated Home Page features authorized services for billable tracking, time tracking and goals/outcome documentation for data entry and review.
