SuccessEHS Client Scenic Bluffs CHCs Earns Highest Level of PCMH Recognition
BIRMINGHAM, Ala., March 7, 2013 /PRNewswire/ --–The National Committee for Quality Assurance (NCQA) recently announced that SuccessEHS client Scenic Bluffs Community Health Centers (CHCs) received the highest level of recognition from its 2011 Patient-Centered Medical Home (PCMH) program. Four of Scenic Bluffs CHCs' providers also earned NCQA 2011 PCMH Level 3 recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and integrating patients as active participants in care. These four providers include: Gloria Degenhardt, PA; Stacey L. Markuson, DO; Elizabeth R. Bade, MD and Lynne Polodna, FNP.
The PCMH care delivery model is designed to strengthen the physician-patient relationship by moving from episodic care to coordinated care and an ongoing relationship with a physician-led care team for wellness and long-term healing. Key domains of the model include improved patient access to care through open scheduling and expanded hours, and strong communication among patients, physicians and staff. Technology plays a key role in achievement.
