Street to Home Named a Mutual of America 2013 Community Partnership Award Winner

NEW YORK, Nov. 21, 2013 /PRNewswire/ -- Mutual of America announced today that Street to Home was named a Merit Finalist in the 2013 Community Partnership Award competition.

Street to Home began 18 years ago, when BronxWorks – founded in 1972 with the goal to lessen the burden of poverty for individuals and families living in the South Bronx – teamed with Montefiore Medical Center (MMC) and Care for the Homeless (CFH) to tackle chronic homelessness in the Bronx. The program combines BronxWorks' social work expertise with MMC's and CFH's medical and behavioral resources.

Services include a 24/7 outreach mobile unit that engages homeless individuals, offering them various shelter options designed to help them move towards self-sufficiency and permanent housing. Dr. Andrea Littleton, the agency's first medical director, accompanies the mobile outreach unit twice a month, providing on-the-spot medical care and prescriptions to the chronically homeless.
