Six Hospital Programs Awarded for Innovative Community Outreach
ATLANTA, Feb. 17, 2015 /PRNewswire/ -- From a medical home for foster children to bringing diabetes to rural areas via telehealth, six hospital programs were named Programs of Promise for their vision to improve the health of local communities. Each program was awarded $2,500 through the Hospital Charitable Services Awards, a national program sponsored by Jackson Healthcare.
The awards were presented at a dinner in Atlanta to representatives from hospital community programs that included urban, suburban and rural regions of the Southeast, Midwest and Northeast.
"We hope these programs will serve as models for health systems and communities seeking unique ways to address underserved health needs," said Charles R. Evans, FACHE, chairman of the Awards program.
The six 2015 Programs of Promise included:
- MetroHealth Medical Home for Children in Foster Care, The MetroHealth System, Cleveland, Ohio. This program serves as the medical home for all 1,600 foster children in Cuyahoga County, providing both routine and sick care.
- Diabetes TeleHealth Network, University of Mississippi Medical Center, Jackson, Mississippi. The program aims to bring critical health care resources to rural areas of the state. Its goal is to serve 200 of the most complex diabetic patients in Sunflower County.
- It's a Girl Thing, Making Proud Choices, Union General Hospital, Farmerville, Louisiana. In less than a year, more than 200 girls enrolled and received resources and support that helps them avoid early pregnancy and STD.
- River Community Wellness Program, River Hospital, Alexandria Bay, New York. The first outpatient program of its kind, this civilian facility offers active duty soldiers coping skills and support to face the impacts of combat.
- The Autism Patient-Centered Specialty Practice, Hospital for Special Care, Hartford, Connecticut. This program specializes in youth living with Autism Spectrum Disorders, pulling together a wide array of education and support for patients and their families.
- The Justice Health Initiative, Cook County Health & Hospital System, Chicago, Illinois. The program targets detainees leaving the Cook County jail and works with them toward improved health outcomes, especially mental health and substance abuse.
Jackson Healthcare instituted the Hospital Charitable Services Awards in 2010 to identify high-impact community health programs, share their stories and offer them as models for other communities to replicate.
This year's six Programs of Programs were selected from a pool of 208 nominees. Each nominee had to demonstrate excellence in the following five criteria areas: community impact, innovation, collaboration, transferability and best practice.
