WOONSOCKET, R.I., June 27, 2014 /PRNewswire/ -- SilverScript Insurance Company, a CVS Caremark (NYSE: CVS) company, announced today that it has entered into collaborations with seven Accountable Care Organizations (ACOs) across the country. The collaborating ACOs include Monarch Healthcare ACO and National ACO in California; American Health Alliance, Orange Accountable Care, Orange Accountable Care of South Florida, and West Florida ACO in Florida; and VirtuaCare ACO in New Jersey. These collaborations are designed to help improve pharmacy care for Medicare Part D beneficiaries who are served by the participating ACO and also enrolled in a SilverScript Medicare Part D Prescription Drug Plan (PDP).
"Our expertise and access to patient prescription data can help these Accountable Care Organizations identify those Medicare Part D patients who could benefit from interventions designed to improve medication adherence, reduce gaps in care and help reduce overall pharmacy spend," said Troyen A. Brennan, MD, Chief Medical Officer, CVS Caremark. "We believe this partnership can help our ACO partners improve key quality measures for their Medicare patients and improve health outcomes in line with our purpose of helping people on their path to better health."
Through the collaboration, clinicians at the participating ACOs will be able to track prescription activity and usage for the SilverScript beneficiaries they treat, including the use of generic versus brand prescriptions and the identification of any medication adherence issues or gaps in care. Based on this information, we will work together to develop and implement appropriate interventions.
As a Medicare Part D PDP sponsor, SilverScript implements a Medication Therapy Management (MTM) program that includes Comprehensive Medication Reviews and other adherence and gaps in care programs that enhance the coordination of pharmacy care for beneficiaries. Through these programs, SilverScript beneficiaries are counseled individually about maintenance medications, patient concerns and any potential risks. These programs are designed to identify concerns around adherence to maintenance medications and possible risks, such as drug-to-drug interactions, duplicate therapies or high risk medications.
These seven Accountable Care Organizations join an existing group of ACOs across the country that SilverScript has been working in collaboration with to improve pharmacy care for the Medicare Part D beneficiaries they serve. These ACOs include PriMed in Connecticut, Franciscan Alliance ACO in Indiana, Jordan Community ACO in Massachusetts, Dartmouth-Hitchcock Health in New Hampshire, and Atlantic ACO in New Jersey.
