The program, in addition to the Seabrook House Foundation Alumni Services, will provide real-time care to patients in their community after discharge from Seabrook House.
Triggr Health is an innovative, easy to use mobile platform that connects users with their care team, provides peer support and enables proactive outreach. Once a patient downloads the application on a smartphone, it utilizes data monitoring and tracks phone behavior to predict the state of someone's recovery in real-time, enabling proactive outreach without relying on self-report methodologies. This risk assessment combines a machine learning algorithm with an individualized profile based on a person's goals, triggers and real life situations to empower their recovery days, months and even years after leaving Seabrook House. The platform allows patients coming out of Seabrook House to get the support they need right when they need it.
If the app detects a user is at risk for a relapse, it informs the Alumni Services Team at Seabrook House. Alumni Services is then able to contact the user to make sure he or she has all the resources needed to stay sober.
Triggr Health also offers access to a peer support group to encourage users through difficult times. It can connect users with other people in recovery and can inform a sponsor, family member or sober coach if needed.
