TO: ALL PERSONS WHO PURCHASED OR OTHERWISE ACQUIRED LHC GROUP, INC. ("LHC") COMMON STOCK BETWEEN JULY 30, 2008 AND OCTOBER 26, 2011, INCLUSIVE (THE "CLASS PERIOD")
YOU ARE HEREBY NOTIFIED that pursuant to an Order of the United States District Court for the Western District of Louisiana, a hearing will be held on December 11, 2014, at 1:30 p.m., before the Honorable C. Michael Hill, at the United States District Court for the Western District of Louisiana, 800 Lafayette Street, Courtroom 6, Lafayette, Louisiana 70501, for the purpose of determining: (1) whether the proposed settlement of the Litigation for the sum of $7,850,000 in cash should be approved by the Court as fair, reasonable, and adequate; (2) whether, thereafter, this Litigation should be dismissed with prejudice against the Defendants as set forth in the Stipulation of Settlement ("Stipulation") dated June 16, 2014; (3) whether the Plan of Allocation of settlement proceeds is fair, reasonable, and adequate and therefore should be approved; (4) whether this Litigation satisfies the applicable prerequisites for class action treatment under Rule 23 of the Federal Rules of Civil Procedure; (5) whether to award Lead Plaintiff's Counsel attorneys' fees and expenses out of the Settlement Fund; and (6) whether to reimburse Plaintiff the costs and expenses (including lost wages) it incurred in prosecuting this action on behalf of the putative Class out of the Settlement Fund.

If you are a Class Member, in order to share in the distribution of the Net Settlement Fund, you must submit a Proof of Claim and Release on or before January 9, 2015, establishing that you are entitled to a recovery. If you desire to be excluded from the Class, you must submit a request for exclusion by November 11, 2014, in the manner and form explained in the Notice. All Class Members who have not requested exclusion from the Class will be bound by the settlement entered in the action even if they do not file a timely Proof of Claim.
