Those affected by Hurricane Irene and Tropical Storm Lee who need assistance in completing their applications can visit one of the Centers during the following hours:

Loan application forms can be downloaded from www.sba.gov. Completed applications should be returned to one of the Centers or mailed to: U.S. Small Business Administration, Processing and Disbursement Center, 14925 Kingsport Road, Fort Worth, TX 76155.
Those affected by the disasters may fill out a loan application online by visiting SBA's website at https://disasterloan.sba.gov/ela/.
The deadline to return SBA applications for physical property damage caused by Hurricane Irene and Tropical Storm Lee is December 15, 2011. The deadline to return economic injury applications for Hurricane Irene is May 31, 2012. The deadline to return economic injury applications for Tropical Storm Lee is June 13, 2012.
