San Francisco Health Plan Announces its New Medication Therapy Management Program to Help Members Avoid Medication Complications
The new initiative will connect our Medi-Cal Seniors and Persons with Disabilities members with trained pharmacists for a complete medication review
SAN FRANCISCO, Nov. 3, 2014 /PRNewswire/ -- San Francisco Health Plan (SFHP), a community health plan serving over 125,000 San Francisco residents, announced its new Medication Management Program to help our Seniors and Persons with Disabilities members get the best results from their medications.
The Medication Therapy Management (MTM) Program, will more closely integrate pharmacists into the health care delivery system. Under the program, members can meet one-on-one with a specially trained Personal Pharmacist™ in their area for a comprehensive medication review.
During this review, the pharmacist will assess all the patient's prescription and non-prescription medications, vitamins, and supplements. If any potential problems are discovered, the pharmacist will work with the patient and his or her doctors to address them. Following the medication review, the pharmacist will provide ongoing services to optimize the patient's medication therapy.
