The RxConcile solution enables pharmacists to connect online via live video and voice chat with discharged long-term care patients and caregivers via a web browser in a fully secure, HIPAA-compliant environment. Medication Mismanagement Top Reason for Medication-Related Readmission
Over 24 million patients requiring some degree of medication counseling are discharged from hospitals each year while nearly 8 Million patients are discharged to post-acute care facilities per year. The national average for hospital readmissions that occur post-discharge from either a hospital or post-acute care setting is more than 23 percent. This means that nearly one out of every four recently discharged patients will return to an institutional care setting following an unsuccessful release to home. Studies suggest that 66 percent of the time, the patient's readmission is the result of some form of medication mismanagement. In addition, a study by retail pharmacy CVS suggests that 50 percent of those medication-related readmissions could be avoided with proper medication counseling and reconciliation.1
