Princeton PO Selects HealthEC® for Population Health
PISCATAWAY, N.J., July 14, 2015 /PRNewswire/ -- HealthEC®, a leading pioneer in population health management solutions, has announced today that it was selected by Princeton Physicians' Organization (Princeton PO) to execute its integrated Population Health Management strategy. HealthEC® was also engaged to provide Princeton PO with an infrastructure solution to establish an Electronic Data Warehouse (EDW) to enhance their efforts to be more clinically and financially integrated.
Over the past six years, the Princeton PO has become and maintained being clinically integrated. As part of their overall improvement processes, they continually investigated and evaluated methods they could use to increase the degree of their integration. As a result of those efforts, their journey led them to discover HealthEC® as a viable solution vendor who could develop their health information exchange (HIE). The goals of the HIE are to enable over 220 of their offices to communicate on clinical matters, as well as provide an enterprise-wide information system to aggregate and provide access to comprehensive medical care plans for all of the Princeton PO members' patients. In turn, this will enable all providers in their clinically integrated network to have a complete picture of each patient's overall health status.
Princeton PO also decided to partner with HealthEC® to assist them on another major initiative: developing their strategy and guiding them through becoming a participant in the Medicare Shared Savings Program (MSSP). Key to this initiative is aggregating all of the data collected from various sources: the insurer, EMR, labs and other providers. Once collected, the data is employed to optimize utilization of resources and achieve the "Triple Aim" of improving both quality of care and patient outcomes, while lowering the overall cost of care.
