ParkTree Community Health Center receives Level 2 Patient Centered Medical Home from the National Committee on Quality Assurance

POMONA, Calif., Sept. 26, 2017 /PRNewswire/ -- Striving for excellence in patient care services, ParkTree Community Health Center (PCHC) has fulfilled its promise to its patients and community through this esteemed recognition. The Patient Centered Medical Home (PCMH) model empowers patients and their families to take an active role in their care and to partner with their health care team to achieve better health outcomes. "When you put quality of care first and enable patients to take responsibility for their well-being, you have the recipe for success," states Carmen Muniz, Chief Operation Officer, who led the health center to achieve this goal. The National Committee on Quality Assurance PCMH recognition program is the most widely adopted evaluation model in the United States. Research shows that implementation of the PCMH model improves quality, reduces cost, improves the patient experience and creates a more positive work environment for the health care team. Achieving PCMH recognition was a large component of ParkTree's 2015-2017 Strategic Plan. Chris Aldworth, Chair of the PCHC Board of Directors, reacted with pride at the accomplishment achieved. 

PCHC is a Federally Qualified Health Center serving the Pomona, Montclair, Chino and Ontario communities. With four health centers to serve, PCHC offers integrated services of medical, dental and behavioral health. In 2016, PCHC served more than 7,600 individuals.
