NOTICE OF POTENTIAL CLASS ACTION SETTLEMENT

VIRGINIA SOLON, as Special Representative for Walter Solon, and CYNTHIA ZALETEL, on behalf of themselves and a class of others similarly situated, v. MIDWEST MEDICAL RECORDS ASSOCIATION, INC. ("MMRA"), JOHN CARDONE, MARK PROTUS, and WILLIAM JACKSON, No. 04-CH-7119, in The Circuit Court Of Cook County, Illinois.

IF you paid for copies of medical records stored in a digital or electronic format from an Illinois health care provider through services provided by MMRA (as a successor in interest to Midwest Medical Records Associates), where such request was made at any time between October 1, 2001 and December 31, 2007, and where such persons and entities paid an amount for per-page copying (not including the statutory handling or processing fee) that is greater than the amount that would have been charged by use of the general, specified  per-page rates set forth in the Illinois Examination of Health Care Records Act, 735 ILCS § 5/8-2001, § 5/8-2003, and § 5/8-2006, subject to certain exceptions, you may be a member of the settlement class entitled to a refund.
