Meridian Health Plan To Integrate Care For Michigan Medicare And Medicaid
DETROIT, Nov. 7, 2013 /PRNewswire/ -- Meridian Health Plan was awarded the opportunity to integrate care for dual-eligible Medicare and Medicaid beneficiaries through the Michigan Demonstration Program to Integrate Care for Persons Eligible for Medicare and Medicaid. Meridian is one of eight plans that will provide the full spectrum of Medicare and Medicaid covered services through its high-quality integrated care delivery model.

Meridian, through experience in existing Medicaid and Medicare subsidiaries, will support the Demonstration by helping to advance care coordination and improve patient care for Michigan residents who are eligible for both Medicare and Medicaid. To qualify for the Demonstration, beneficiaries must be eligible for Medicaid benefits, Medicare Parts A and B and reside in the awarded service area that includes Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph and Van Buren counties. The Michigan Demonstration Program is subject to federal CMS approval.
