Los Angeles CCTP Earns Renewal with Support from Loopback Analytics
Partners in Care Foundation Harnesses Loopback's Data Integration & Workflow Platform to Support Local Hospitals, Engage Nearly 900 Patients a Month to Improve Quality of Care & Reduce Hospital Readmissions

Partners' Westside CCTP supports patients from three hospitals, representing two hospital systems, while the Glendale CCTP supports patients from three hospitals, representing two systems.
