FRANKLIN, Tenn., Sept. 13, 2017 /PRNewswire/ -- i2i Population Health, a national leader in population health management (PHM) technology and 2017 Best in KLAS category winner, welcomes KeyCare as the newest Health Center Controlled Network (HCCN) to implement i2i's solutions. KeyCare and i2i have officially launched the implementation program that will greatly contribute to clinical and operational improvement at the sixteen participating New Jersey Community Health Centers (CHCs) in the current HRSA HCCN grant project. Five of the sixteen centers are currently using i2i and will benefit by deploying additional PHM solutions as being part of the network.
