Home State Health Announces Partnership with MissouriHealth+
ST. LOUIS, Nov. 18, 2014 /PRNewswire/ -- Home State Health, the Missouri subsidiary of Centene Corporation (NYSE: CNC), today announced a collaborative performance incentive agreement with MissouriHealth+ which enhances access to quality health care and improves health outcomes for children and families enrolled in the MO HealthNet managed care program.
Since its inception, Home State Health has worked closely with the local community health centers located in the managed care service area. To help improve access and quality health outcomes, Home State Health provides real-time actionable information to its primary care physicians, which highlights patient health risks and gaps in preventable and screening services. Under the new agreement with MissouriHealth+, physician performance incentives resulting from improved health outcomes will be extended to all of the Federally Qualified Health Centers (FQHCs) that are part of MissouriHealth+, representing over 500 providers across 170 locations.
"Home State Health supports whole-person care coordination and by collaborating with Missouri's FQHCs, we've developed an innovative agreement which provides resources and incentives to help primary care physicians better manage the overall health of their patients" said Shannon Bagley, President and CEO for Home State Health.
"We are excited to announce this partnership and expand the relationship between Missouri's community health centers and Home State Health" said MissouriHealth+ CEO Danny O'Neill. "MissouriHealth+ and Home State Health will collaborate to provide information to our providers so they can assist patients to make good decisions and achieve better health outcomes. MissouriHealth+ centers have invested time and resources into their operations and this network. We are pleased that Home State Health continues to recognize the quality care delivered by community health centers."
"Providing the best possible care to our members requires innovative thinking and collaboration among leaders in the healthcare industry" said Ms. Bagley. "This agreement is an example of what can happen when stakeholders come together to produce real results for Missouri families."
