SAN FRANCISCO, Dec. 16, 2010 /PRNewswire/ -- New participants to San Francisco's Healthy San Francisco program may now choose Brown & Toland Physicians when selecting their medical home. Healthy San Francisco is a city wide program which provides access to basic medical services for uninsured residents in San Francisco.

Brown & Toland has partnered with California Pacific Medical Center (CPMC) to provide a medical home in Healthy San Francisco's network. Brown & Toland will provide primary care and specialty services to program participants while CPMC will serve as a network hospital. People who qualify for this program may select Brown & Toland Physicians-CPMC as their medical home beginning December 1.
