Health Diagnostic Laboratory, Inc. Partners with Patient-Centered Care Collaborative
RICHMOND, Va., Oct. 13, 2011 /PRNewswire/ -- Health Diagnostic Laboratory Inc. (HDL, Inc.), a CAP accredited leader in health management, announced today that it is the first advanced laboratory to join the Patient-Centered Primary Care Collaborative (PCPCC) executive committee. The PCPCC, created in late 2006, is one of the major developers and advocates of the patient centered medical home (PCMH) model in America. The Collaborative membership includes a number of large national employers, major primary care physician associations, health benefits companies, trade associations, profession/affinity groups, academic centers, and healthcare quality improvement associations.
PCMH's approach incorporates the whole person and self-care, where patients with chronic health conditions, like cardiovascular disease, are proactively supported in making lifestyle changes for prevention and wellness rather than being treated for symptoms of disease.
