In addition to 42 Skilled Nursing facilities, Grace also offers Assisted Living, Rehabilitation and Alzheimer/Memory Care. The COMS implementation will focus initially on the Grace Skilled Nursing facilities.
The COMS product suite, Daylight IQ™, provides care information at the bedside for clinical teams. The Software as a Service (SaaS) product significantly empowers the entire care team including physicians, nurses, nursing supervisors and nurse aides, and facilitates improved communication between all stakeholders. Key features of the product suite include an Automated Disease Care Guide Library, a Dynamic Nursing Assessment and Compare Module, all of which work in conjunction with each resident's disease profile. An important component of the Daylight IQ™ program is a reduction in avoidable hospital readmissions by more than 50%.
