The webinar, "Rural Practice Transformation: Getting Ready for Value-Based Payments" is scheduled for January 5, 2016 at 12 Noon ET and January 9, 2016 at 2 pm ET. There is no charge to participate and pre-registration is not required. To join the webinar, call 303.248.0285 / 5004770#, and log in via www.readytalk.com / code 5004770.
This webinar will provide an overview of a new federally funded program for rural providers, Practice Transformation Networks (PTNs), which provide the necessary framework for improving population health and lowering overall costs while improving the financial sustainability of rural health systems.
Barr explains, "Rural providers must get ready for the movement to value-based payments but often aren't sure where to start and don't always have internal resources available. The federally-funded Practice Transformation Network provides a clear path to follow and also funds the necessary infrastructure and support."
Participants in the Consortium's program receive intensive training to improve quality and patient satisfaction and reduce unnecessary costs. Federal funds pay for this training, in addition to the IT infrastructure, analytics, Advice Nurse hotline, patient satisfaction surveys, clinically integrated networks and other essential elements to be successful under the new Value-Based Payment Models. There is no cost for rural providers to join the program. Upon completion of the program, rural providers will be prepared for participating in the value-based payment model of their choice, including ACO's, bundled payments, and Patient Centered Medical Homes.
