MILWAUKEE, Nov. 15, 2018 /PRNewswire/ -- Approximately 10,000 people cared for by the Froedtert & MCW health network have used PatientWisdom, a mobile-responsive digital site which helps patients share 'stories' about what matters to them, to improve health and care by listening, individually and at scale.
People are using this secure web-based tool to let providers know what matters to them ahead of clinical encounters to improve patient experience and the delivery of care. A one-screen summary with what matters to each individual, such as health priorities, goals, barriers to staying healthy, pressures, and worries, is delivered via the electronic health record to every member of the care team ahead of the patient visit.
From the perspectives people share, care teams can more fully and quickly understand how to deliver care that is individualized for the whole person. Froedtert & MCW is one of the first health systems in the world to offer a digital health tool like PatientWisdom.
