Evolent Health Partners with Legacy Health to Enhance Care Delivery in the Pacific Northwest

WASHINGTON, Nov 5, 2015 /PRNewswire/ -- Evolent Health, the leader in helping progressive health care systems accelerate the path to value-based care, announced today its long-term partnership with Legacy Health. The multi-year partnership will guide the development and execution of a value-based care strategy for improving patient health in the Portland-Vancouver region.
