Evangelical Homes of Michigan Launches New Service - LifeChoices(TM) - and Subsidiary - LifeChoice Solutions(TM)
ANN ARBOR, Mich., April 27 /PRNewswire-USNewswire/ -- Evangelical Homes of Michigan is launching a new service called LifeChoices™, which is a continuing care at home program, or sometimes referred to as a virtual retirement community. Starting in May 2010, we will provide members a variety of products and services to allow them to remain independent in their own homes while securing their future. Evangelical Homes of Michigan is 1 of 5 service providers nationwide that offers a continuing care at home program.
LifeChoices™ is being offered through LifeChoice Solutions™, a wholly owned subsidiary of Evangelical Homes of Michigan. LifeChoices™ combines the high-quality care and broad range of services provided by Evangelical Homes of Michigan with the security and peace-of-mind offered by a traditional continuing care retirement community all within the comfort of the client's home. This new program is being led by Stephen Hopkins, Executive Director, LifeChoice Solutions™.
After signing up for LifeChoices™, members are charged a one-time membership fee and an ongoing monthly fee. LifeChoices™ members are provided a lifestyle coach that will help them to navigate the all too confusing health care systems and services. Their coach will match needed services to the clients within their own home. The program allows the client to purchase future services at today's rates, providing them with the security they desire. As a client's health conditions change they will already be covered. Considering the growing population of individuals over the age of 65, as well as limited access to services and rising costs of Long-Term Care, LifeChoices™ members can live comfortably knowing that their future is already secure.
Membership includes 100% coverage for all core services, including wellness & education, medical options, advocacy & care management, hospitality & environmental services, and some home & community based services, and technology. All members must be at least 60 years of age, live independently within Washtenaw, Wayne or Oakland Counties, maintain Health Insurance and meet medical diagnostic criteria. There is no maximum age for members. Memberships can work with individuals' existing Long-Term Care insurance policies. Memberships for the LifeChoices™ program will be available starting in May 2010.
