The new form will help standardize definitions and create a process that captures all essential data related to overdoses in a simple, easy-to-use format while eliminating duplicate entry of information. Additionally, the form is designed to highlight issues related to the well-being and safety of providers by identifying any injury-related events associated with a response.

The Surveillance Form for Overdose form will track any type of overdose, from alcohol to acetaminophen to stimulants – including opioids. This new form is the first in a series that will provide a template for additional surveillance activities, including infectious diseases, stroke, biohazards, etc., for the purpose of gathering data and identifying trends around multiple types of response events.
Guilford County Emergency Services in Greensboro, North Carolina was one of the first agencies to use the Surveillance Form for Overdose. The EMS agency is already recognizing the potential for the form.
"The information gathered from this form will help us understand what types of overdoses we are responding to, and more importantly, who those patients are demographically so we can improve our response to these calls," said David Moran, EMS Management Consultant. "The form will give us another tool to use for understanding where overdose calls are occurring within our response areas as well as identifying clusters and other potential safety concerns for the responding medics to consider."
