EmblemHealth to Offer Managed Long-Term Care Benefit to Help Members Maintain Independent Living in Their Community
NEW YORK, Aug. 3 /PRNewswire/ -- The New York State Department of Health has approved HIP Health Plan of New York (HIP), an EmblemHealth company, to operate a Medicaid Advantage Plus Long-Term Care plan for dual eligible Medicare and Medicaid individuals. The HIP VIP Dual Eligible (HMO) Medicaid Advantage Plus-Managed Long-Term Care (MLTC) plan is designed to allow members to maintain their independence in the community for as long as possible at no additional cost.

"The goal of the MLTC program is to assist frail, elderly and disabled individuals so that they can remain in their homes for as long as possible."
The MLTC program will be offered in all five boroughs of New York City, as well as Westchester, Nassau and Suffolk Counties. Every participant will be assigned a Nurse Care Manager who will work with network physicians, nurses, social workers and other health care providers to develop a personalized plan of care.
In addition to their existing benefits, members of the MLTC program will receive personal care; help with chores, housekeeping and cooking; home and adult-day health care; social day care; delivered meals; and personal emergency response systems.
Enrollment begins in August, and benefits will go into effect on September 1, 2010.
To be eligible, applicants must enroll in the HIP Medicare Advantage Product, be eligible for both Medicare and Medicaid, be 18 years of age or older, reside in the service area and require care services to assist with daily living that meet Department of Health criteria.
