SAN FRANCISCO, April 4, 2017 /PRNewswire/ -- Elation Health announced today its efforts to support independent primary care physicians that are participating and interested in participating in the Centers for Medicare and Medicaid Services (CMS) Comprehensive Primary Care Plus (CPC+) program. Elation Health is a Clinical First electronic health record dedicated to helping independent primary care physicians deliver collaborative, patient-centered care. CPC+ is an advanced primary care medical home model that offers incentives for delivering high-quality primary care, and has been dubbed the "largest-ever" move to change US primary care delivery and payment. The program launched in January 2017 for nearly 3,000 participating practices and recently, CMS announced a second round of the program.
Elation Health has pledged to support CPC+ practices for the full five-year duration of the program. Many of the physicians using Elation's electronic health record are primary care practices in the 14 regions that were chosen for the initial launch of CPC+. The program gives physicians the opportunity to receive incentives for delivering exceptional primary care and provides a path for independent primary care practices to transition to value-based reimbursement while earning recognition for the outstanding care they deliver.

Elation Health has invested in the necessary tools and support to help participating physicians succeed in the CPC+ program. In addition to technology support, Elation has launched a guide to the program within its resource center, guided webinars to answer questions, and an on-demand health policy team.

The second CPC+ performance period begins January 1, 2018. After proposals from payers in new regions and payers in the existing 14 CPC+ regions are reviewed, CMS will announce new regions and payer partners. Practices located in up to 10 new CPC+ regions can apply in Summer 2017 with a certified EHR to be eligible for the program.
