KNOXVILLE, Tenn., Sept. 20, 2017 /PRNewswire/ --
WHAT: |
Now in its fifth year, Medicare's Hospital Readmissions Reduction Program (HRRP), imposed under the Affordable Care Act, has aimed to encourage hospitals to reduce unnecessary and avoidable patient readmissions. Kaiser Health News has reported Medicare is punishing 2,573 hospitals nationally, just two dozen short of what it did last year, according to federal records. Starting in October, the federal government will cut those hospitals' payments by as much as 3 percent for a year. |
Nurses and nurse case managers can play a pivotal role in reducing readmissions, through efficient coordination, communication, planning and education. In this free webinar hosted by PerfectServe®, Eric A. Coleman, M.D., MPH, will familiarize attendees with HRRP and its impact to date, and share insights on data, measures and other considerations that affect or prevent readmissions relative to nurses supporting the care transition process. He will review proven strategies, programs and interventions to reduce readmission rates and improve the patient experience. |
