Dr. Eric Coleman Headlines PerfectServe's Webinar Presenting Care Transition Strategies to Help Nurses Improve Readmission Rates

WHAT:

Now in its fifth year, Medicare's Hospital Readmissions Reduction Program (HRRP), imposed under the Affordable Care Act, has aimed to encourage hospitals to reduce unnecessary and avoidable patient readmissions. Kaiser Health News has reported Medicare is punishing 2,573 hospitals nationally, just two dozen short of what it did last year, according to federal records. Starting in October, the federal government will cut those hospitals' payments by as much as 3 percent for a year.

Nurses and nurse case managers can play a pivotal role in reducing readmissions, through efficient coordination, communication, planning and education. In this free webinar hosted by PerfectServe^®, Eric A. Eric A. Coleman, M.D., MPH, is Professor of Medicine and Head of the Division of Health Care Policy and Research at the University of Colorado Anschutz Medical Campus. Dr. Coleman is the Director of the Care Transitions Program, aimed at improving quality and safety during times of care "handoffs." He also serves as the Executive Director of the Practice Change Leaders. Dr. Coleman was recognized with a 2012 MacArthur Fellowship for his work in bridging innovation and practice through enhancing the role of patients and family caregivers in improving the quality of their care transitions.
