BOSTON, July 18, 2012 /PRNewswire/ -- The Lucian Leape Institute (LLI) at the National Patient Safety Foundation has announced the program for its 5th Annual Forum & Gala to be held in Boston on Thursday, September 13, 2012.

Attendees will engage in an interactive afternoon dialogue with LLI members, followed by an evening networking reception and dinner featuring guest speaker Donald Berwick, MD, MPP, FRCP, former president and CEO of the Institute for Healthcare Improvement and former administrator, Centers for Medicare & Medicaid Services.
