NEWARK, Calif., Nov. 13, 2023 /PRNewswire/ -- CymaBay Therapeutics, Inc. (NASDAQ: CBAY), a biopharmaceutical company focused on innovative therapies for patients with liver and other chronic diseases, today announced that the results from its RESPONSE Phase 3 registration study evaluating seladelpar, a potent, selective, orally-active delpar, or PPAR𝛿 agonist, in adult patients with primary biliary cholangitis (PBC) will be featured as an oral presentation in today's Late Breaker Session of the American Association for the Study of Liver Diseases' (AASLD) The Liver Meeting® 2023 in Boston, Massachusetts.
RESPONSE was a double-blind, placebo-controlled, global study of one-year duration that randomized 193 PBC patients in a 2:1 ratio to seladelpar 10 mg or placebo, once daily. Eligible patients had an inadequate response or intolerance to ursodeoxycholic acid (UDCA) with serum alkaline phosphatase (ALP) ≥ 1.67× the upper limit of normal (ULN) after at least 12 months of treatment. The primary endpoint was a composite of ALP and total bilirubin previously accepted by the U.S. Food and Drug Administration (FDA) and European Medicines Agency (EMA) for registration studies in PBC. The composite endpoint was achieved in 61.7% of patients on seladelpar vs. 20.0% on placebo (p<0.0001).
The key secondary endpoint of ALP normalization occurred in 25% of patients receiving seladelpar vs. 0% for patients on placebo (p<0.0001). The average decrease in ALP for patients on seladelpar was -133.9 U/L vs. -16.9 U/L for patients on placebo (p<0.0001). Compared to placebo, seladelpar reduced alanine aminotransferase (ALT) at Month 12 by 23.5% vs. 6.5% (p=0.003) and gamma-glutamyl transferase (GGT) by 39.1% vs. 11.4% (p<0.0001).
The study also measured change in patient-reported pruritus (itching), one of the most challenging symptoms experienced by patients with PBC, as a key secondary endpoint using the daily numerical rating scale (NRS; 0-10). The pruritus endpoint was met at Month 6 among patients with baseline NRS > 4 reporting decreases of 3.2 points with seladelpar (n=65) vs. 1.7 for patients on placebo (n=20; p<0.005). Notably, these improvements were sustained through Month 12 (p<0.005). Approximately 37.3% of patients enrolled had moderate-to-severe pruritus based on an NRS (0-10) score ≥ 4 at baseline. A statistically significant reduction in pruritus was also observed at Month 6 and at Month 12 for patients in the intent-to-treat population, which includes all patients irrespective of their NRS score at baseline.

The potency, efficacy biochemically, and significant anti-pruritus effects, alongside the safety profile of seladelpar, really does reinforce its potential to change the treatment landscape in PBC."
There were no treatment-related serious adverse events in the study. Discontinuation due to adverse events occurred in 3.1% of patients taking seladelpar vs. 4.6% on placebo. Long-term safety and tolerability are being evaluated in the open-label ASSURE trial, which currently has over 300 patients enrolled (NCT03301506).
These results (Abstract #5002) will be shared in a presentation titled "Efficacy and Safety of Seladelpar in Patients with Primary Biliary Cholangitis in the RESPONSE Trial: A Phase 3 International, Randomized, Placebo-Controlled Study," on November 13, 2023, as part of the late-breaking oral presentation at 2:30 PM EST. The results will be presented by Dr. Hirschfield on behalf of the RESPONSE Study Investigators.

PBC is a rare, chronic inflammatory liver disease primarily affecting women (1 in 1,000 women over the age of 40 or about 130,000 total people in the US).
