The use of Valera's technology to support behavioral health activation and self-care, using a smartphone-based platform that leverages data analytics, will be a novel intervention for this FQHC serving patients with high-needs and complex social conditions. Valera's technology will offer the ability to collaborate between teams and monitor patients.
