PLANO, Texas, Dec. 15, 2016 /PRNewswire/ -- Community Health Network/Indiana ProHealth Network (CHN/IPN), which is ranked as one of the most integrated healthcare systems in the United States, has become the newest ZeOmega^® health system client. CHN/IPN also plans to leverage the unmatched capabilities inherent in the end-to-end Jiva™ platform to integrate data, streamline efficiencies, and improve collaboration.

CHN/IPN comprises an integrated network of more than 200 sites of care and affiliates throughout Central Indiana. The nonprofit health system will implement Jiva's advanced case, disease, and utilization capabilities to achieve its goal of providing convenient access to exceptional healthcare services, where and when patients need them.
