Community Care Partners and Meridian Health Plan Unite to Improve Patient Health
New partnership becomes Illinois' largest and highest quality-rated Medicaid HMO
CHICAGO, Feb. 23, 2016 /PRNewswire/ -- Community Care Partners (CCP) will be partnering with Meridian Health Plan to better serve the growing population of Medicaid beneficiaries in northern Cook and Lake Counties. This will take effect May 1, 2016. The 41,000 CCP members will not experience any change or disruption in care.
NorthShore University HealthSystem (NorthShore) created CCP as an Accountable Care Entity (ACE) in 2014 and included three other regional healthcare providers that remain committed to improving efficiency and access to quality care. CCP originally partnered with the State of Illinois as an ACE, but changes in the state's Medicaid program led CCP to join Meridian Health Plan.

CCP is comprised of NorthShore, Erie Family Health Center, Lake County Health Department and Community Health Center and Vista Health System. All four will continue to deliver high-quality and coordinated care to members, only now as part of Meridian Health Plan.
