ATLANTA, June 9, 2016 /PRNewswire/ -- Wellcentive, a leader in value-based care solutions, today announced that five pediatric sites within the Children's Health Alliance have been recognized by the state of Oregon for achieving its highest patient-centered medical home (PCMH) rating as Tier 3 STAR practices.
"Population health management has been a critical component of the success of the practices within our clinically integrated network," said Julie Harris, Children's Health Alliance director of quality programs. "Wellcentive's platform has enabled these pediatric practices to transform their approach to delivering proactive, patient-centered care."
To date, only seven practices in Oregon have achieved the Tier 3 STAR rating. Of those, five are independent pediatric practices and members of the Children's Health Alliance.
"CHA has been a beacon of change for pediatric care, and we congratulate our partners at the Alliance for this recognition," said Tom Zajac, Wellcentive CEO. "We are honored to support their transformative efforts both at the practice level and across the network's vision of enhanced care. Their medical home model is a great example of the movement to patient-centric care and care coordination as central elements of population health management, driving advanced payment models and responding to new MACRA requirements."
The Alliance practices that have achieved Tier 3 STAR ratings include:
- Childhood Health Associates of Salem
- Metropolitan Pediatrics – Gresham
- Metropolitan Pediatrics – Happy Valley
- Metropolitan Pediatrics – Northwest
- Metropolitan Pediatrics – Westside
