CareCentrix's Chief Medical Officer to Present Hospital Readmission Reduction Program at World Congress Leadership Summit
HARTFORD, Conn., Jan. 27, 2011 /PRNewswire/ -- CareCentrix, Inc., the nation's leading provider of home health benefits management services, will present tomorrow at the World Congress' 2nd Annual Leadership Summit on Hospital Re-Admissions in Washington, D.C. Dr. William Kerr, Chief Medical Officer, will discuss CareCentrix's HomeSTAR™ programs which are reducing hospital readmissions through the use of home based interventions.
Hospital readmissions are estimated to cost the U.S. health care system $31 billion, according to an April 2009 study by the U.S. Agency for Healthcare Research and Quality. Twenty percent of all Medicare patients are estimated to return to the hospital within thirty days of discharge, and fifty-six percent will be readmitted within a year, according to a January 2009 study reported in the New England Journal of Medicine. To address these findings, CareCentrix has developed a structured program providing discharged patients with transitional, home-based support services. These services have proven to increase the likelihood of a successful recovery at home, avoiding a readmission to the hospital. Services are delivered by home health agencies educated in the program and chosen from CareCentrix's nationwide network. The HomeSTAR™ name describes the program's goal to provide patients a Home-based Successful Transition And Recovery from their in-patient stay.
