OKLAHOMA CITY, Aug. 21, 2014 /PRNewswire/ -- The Coalition of Advocates for Responsible Eldercare (CARE Oklahoma) has announced their newest CARE Partnership with COMS Interactive, LLC (also known as Clinical Outcomes Management Systems or COMS). The two organizations announced the signing of a Gold Level Partnership agreement which establishes COMS as a CARE Preferred Partner. Oklahoma facilities, and their 20,000 employees provide care for approximately 20,000 elderly and disabled individuals in Skilled Nursing and Assisted Living Facilities throughout the State.
The COMS flagship Disease Management software, Daylight IQ™, has been successfully deployed in Skilled Nursing and Assisted Living facilities and Home Care organizations throughout the US. The Software as a Service (SaaS) product features a series of integrated, disease-based clinical protocols which significantly empower the entire nursing team at the point of care, while facilitating enhanced communication between all parties. Daylight IQ™ training can be completed in less than two hours, immediately improving clinical and financial outcomes.
"COMS provides progressive leadership and innovation required to elevate care for the elderly and frail to the next level," commented Keith Dobbs, President and CEO of CARE Oklahoma. "They have demonstrated quality enhancements in facilities throughout the US, and we are anxious for them to provide assistance to Oklahoma care providers."
Daylight IQ™ empowers clinical teams with information and technology resulting in caregivers providing better care for residents. A key component of Daylight IQ™ is the reduction in unnecessary hospital re-admissions by as much as 50%, a related decrease in premature mortality rates, and an increase in successful discharges.
"We are pleased to partner with CARE Oklahoma," stated Edward J. Tromczynski, Chief Executive Officer, COMS Interactive. "This progressive State Association continues to demonstrate thought leadership by focusing on improving clinical outcomes for those in their care, and we look forward to working with the CARE team and members."
Market information indicates that the average Skilled Nursing Facility, Home Care or Assisted Living resident has a complicated disease profile, with one primary disease and up to eight secondary diseases or afflictions. Over 70% of re-hospitalizations are due to the worsening of secondary or new diseases. By providing early detection of even minor changes in condition, Daylight IQ™ highlights potential problems and offers caregivers the opportunity to respond, preventing further progression of the illness.
