Executives at one of Michigan's largest healthcare providers began earlier this year to automate their approach to population health for chronically ill patients, all with the goal of engaging more patients to better manage their disease. The results of this new way of thinking have been dramatic.
In this session, Gaines will explain how her care team implemented this new approach across Borgess Health – including using Wellcentive's key tools for tracking, monitoring and reporting patient information – and how it helped improve the health of at-risk diabetes patients across 10 counties despite living with a chronic condition.
Gaines will compare the previous method for identifying diabetes patients, a lengthy and manual process, to the current automated approach that utilizes an electronic health record interfacing with Wellcentive's population health management solution. Wellcentive enabled the rapid identifying of both at-risk patients and the gaps for improvement leading to treatment insights.
Presented are examples of successful patient engagement results including lowering the percentage of diabetes patients with hemoglobin A1c greater than nine. By analyzing data in the patient registry, Borgess Health discovered many patients hadn't had their hemoglobin A1c checked in over 12 months. These patients were quickly enrolled in care management programs, and within three months, their hemoglobin A1C greater than nine dropped from 30 to 19.6 percent. Plans are under way to apply this approach to additional patient populations across Michigan.
