NEW YORK, Dec. 2, 2014 /PRNewswire/ -- Experts from AMC Health, a healthcare IT company recognized for advancing monitored care, will illustrate various positive impacts of telemonitoring during presentations at the upcoming mHealth Summit. The Summit will be held from December 6-11, 2014, at the Gaylord National Resort, National Harbor, MD. AMC Health and Geisinger Health Plan presenters will share how mobile telemonitoring applications have improved clinical outcomes, reduced readmissions and lowered the cost of treating patients with chronic conditions. She will share information about the Visiting Nurse Service of New York's (VNSNY) program with AMC Health that incorporated tailored biometric monitoring and automatic symptom capture for at-risk patients discharged with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), heart attacks and diabetes. She will detail the challenges, program design and results that included a significant reduction in readmissions, dramatically increased efficiencies, and improved physician engagement.
Reducing CHF Hospitalization and Cost of Care - Sunday, Dec. 7th, 11:45 am-12:30 pm, Room National Harbor 12-13 – Doreen M. Salek, Director of Population Health Business Intelligence for Geisinger Health Plan (GHP), and Joanne Sciandra, RN, BSN, CCM, Associate VP, Population Management, Geisenger Health Plan, will present: "Can Telemonitoring Reduce Hospitalization and Cost of Care? A Health Plan's Experience in Managing Patients with Heart Failure." Their presentation will be based on a peer-reviewed study published in Population Health Management that determined, among other findings, that the long-term program with AMC Health, conducted between 2008 and 2012, generated a 3.3 to 1 return on investment -- for every $1 spent to implement this program, GHP saved about $3.30, which translated to an 11 percent reduction in cost of care per patient per month.
Improving Clinical Trial Efficiency with Tele-video and Remote Patient Monitoring - Tuesday, Dec. 9th, 11:00 am-12:00 pm, Room National Harbor 4-5 – John Larus, Senior Vice President, Solutions Development for Clinical Trials, AMC Health will serve as a panelist on "NEW NEW THING! Beyond the Pill: mHealth in Pharma & Pharmacy." As part of the discussion about the uses and benefits of deploying mHealth along the drug development spectrum, Larus will share information and insights about the impact of initiatives AMC Health has already implemented (including video visits and remote monitoring) on the efficiency and cost of clinical trials.
Patient Monitoring in the Medical Home - Tuesday, Dec. 9th, 3:45 pm-4:45 pm, Room National Harbor 2-3 – Jonathan Shankman, Senior Vice President, Clinical Innovation, AMC Health, will participate in the session, "Designing the Medical Home: Meeting the Needs of Consumers and Providers." In this session, medical home solution providers will discuss their products and address their approaches and specific product features designed to meet the divergent needs of consumers and providers, and will provide advice on how to evaluate products for the medical home.
