Alexandra Berg Appointed Director Of Transitional Care For New Able Care Connect TCM Business For The Goodman Group
LARGO, Fla. and CHASKA, Minn., Oct. 7, 2014 /PRNewswire/ -- The Goodman Group, a national leader in developing and managing senior living and health care communities, today announced the appointment of Alexandra (Alex) Berg, MSN, ARNP, a board-certified adult nurse practitioner, as director of transitional care for Able Care Connect. Able Care Connect focuses solely on providing Transitional Care Management (TCM) clinical service. TCM was developed by Medicare to improve the continuum of care for older adults who need additional assistance after a hospital discharge and helps prevent the potential risk for hospital readmissions.
Berg will work with older adults as they transition across health care settings to maintain communication, oversee medication management, improve understanding of health care plans, and assist with community referrals when needed. This Transitional Care Management will be customized to help individuals meet their health care goals and reduce the need for hospitalizations. TCM is a reimbursed service approved by Medicare, and accepted by many other large insurance carriers for qualified participants.
Her appointment is effective immediately and she will be based at the 95-acre, intergenerational Palms of Largo senior living & health care community, developed and managed by The Goodman Group. The appointment was announced by Craig Edinger, vice president, senior living & health care division, The Goodman Group.

Alex is a Largo, Fla. native. After graduating from Largo High School, she received a Bachelor's of Science in Nursing from Vanderbilt University and a Master's of Science in Nursing from University of Virginia, and went on to complete a nurse practitioner certificate program at Emory University. She has been a nurse practitioner since 2001.
