SALT LAKE CITY, Nov. 3, 2017 /PRNewswire-USNewswire/ -- Collective Medical, delivering the nation's largest network for care coordination, is pleased to announce Alaska's statewide implementation of Collective Medical's platform to support improved patient outcomes. This effort is a result of Collective Medical's collaborative partnership with The Alaska State Hospital and Nursing Home Association (ASHNHA), the State of Alaska, and the Alaska Chapter of the American College of Emergency Physicians (ACEP).
"We are thrilled to partner with ASHNHA, the Alaska Chapter of ACEP and the State of Alaska on this effort support of care teams everywhere," says Chris Klomp, CEO of Collective Medical. "No patient should ever slip through the cracks across healthcare settings. This is another step in achieving that vision by enabling providers to seamlessly and immediately collaborate across the full continuum of care, ensuring the best possible health outcome for each patient."
Collective Medical's network and platform is proven to increase the effectiveness of existing care management resources, reduce medically-unnecessary emergency department (ED) admissions and hospital admissions, and improve quality through the consistent delivery of patient care.
"We are enthusiastic about our partnership with CMT to reduce the unnecessary use of emergency rooms, not just for the Medicaid population, but for all ED users" says Becky Hultberg, president and CEO of ASHNHA. "Our partnership with Collective Medical is one step towards improving health for all Alaskans."
PreManage ED, part of Collective Medical's PreManage platform, integrates within existing clinical workflows—including the ED track board—to push actionable insights to ED providers the moment a high-risk patient presents. This gives the provider immediate perspective on the patient without having to search through voluminous clinical records. The platform does this by synthesizing key insights including ED visit history, community-sourced care plans, and other valuable clinical and social history information.
Anne Zink, MD, FACEP, medical director with Mat-Su Emergency Physicians, comments that "having a system allowing us to communicate in real time on ED visits and care plans for complex patients has been a dream in Alaska for years." She adds that, "we're excited Alaska has partnered with Collective Medical to provide real-time patient information across a multi-state network. Alaska ACEP is committed to serving all patients in the state of Alaska as we strengthen the safety net for the most vulnerable in our population."
