Wellcentive to Empower Two New Pediatric Population Health Programs at Seattle Children's Hospital
Clinically integrated network at leading children's hospital will include an innovative care management program for disabled children.
ATLANTA, Oct. 13, 2015 /PRNewswire/ -- Wellcentive, a leader in value-based care and population health management solutions, announced today that it will empower two new innovative initiatives in Pediatric Population Health Management: Seattle Children's Care Network (SCCN) and Pediatric Partners in Care (PPIC), both at Seattle Children's Hospital.
Through a three-year contract, Wellcentive will deliver its population health management and value-based care solutions to support SCCN, Seattle Children's recently developed Clinically Integrated Network; and PPIC, a program funded by a $5.56 million CMS Innovation Center Grant that aims to improve health outcomes and reduce costs of care for some 3,000 disabled children with multiple medical issues.
