ViiMed® contracts with Johns Hopkins HealthCare to bring digital health solutions for diabetes to hospitals, health systems, employers, payors, and patients
WASHINGTON, Dec. 14, 2017 /PRNewswire/ -- ViiMed, a Washington, D.C.-based digital health company, announces that it has signed a five-year contract with Johns Hopkins HealthCare LLC, a division of Johns Hopkins Medicine to convert their evidenced-based health solutions to digital form and offer them to hospitals and patients.
Digital health solutions on the ViiMed Platform share a common goal – improve patient outcomes by virtually engaging, managing, monitoring, and coordinating the patient and care team in and out of the health care setting. The focus of the two initial Johns Hopkins programs is on changing behavior of individuals with either pre-diabetes or type 2 diabetes.
The DECIDE program helps people with diabetes learn to change their behavior so they can better self-manage. This unique interactive program improves both self-management skills and diabetes health outcomes. Developed by Felicia Hill-Briggs, Ph.D., senior director of Population Health Research and Development at Johns Hopkins HealthCare, professor of medicine at the Johns Hopkins University School of Medicine, DECIDE has shown positive outcomes in studies of high risk, low-literacy populations. In addition, the program has been shown to improve A1C levels, blood pressure and cholesterol.
ACT2 is an interactive, year-long support and engagement program that is designed to empower those at high-risk of diabetes to take charge of their own health and well-being. Based on the Centers for Disease Control and Prevention's National Diabetes Prevention Program curriculum, ACT2 will combine education, remote monitoring, and behavior change into one program for those diagnosed with pre-diabetes. In 2018, the Centers for Medicare & Medicaid Services is scheduled to announce performance-based reimbursement for diabetes prevention, and ACT2 will seamlessly enable organizations to implement this program into their workflow.
