Urgent Care Provider CityMD Announces NCQA Recognition
CityMD is the first urgent care to receive this award
NEW YORK, April 19, 2016 /PRNewswire-USNewswire/ -- CityMD Urgent Care announced today that it is the first NY urgent care to receive NCQA Patient-Centered Connected CareTM Recognition in the nation and is the first privately owned urgent care practice to receive this recognition.
CityMD's locations receiving this recognition are the Yonkers, Riverdale and White Plains sites.

NCQA Patient-Centered Connected Care Recognition is awarded to sites that provide outpatient care for delivering intermittent or outpatient treatment, but does not act as the patient's primary care provider. The program is a roadmap for delivering evidence-based, patient-centered care that focuses on closing the gaps in healthcare. It requires urgent cares to convey relevant patient information to a patient's primary care physician. Requirements also include support communications with secondary providers for patients receiving long-term specialty care.
CityMD prides itself on helping patients find their way back to their medical home with their primary care doctor or specialist. This recognition highlights CityMD's standards of practice, continued focus on gaining the trust of its providers and resulting in better outcomes and improved patient experience.
The standards examine:
- Connecting with primary care: The site connects with and shares information with patients' primary care providers and helps them find a primary care provider if they don't have one.
- Identifying patient needs: The site directs patients to appropriate providers when necessary.
- Patient care and support: The site uses evidence-based decision support in care delivery, collaborates with patients to make care decisions and delivers culturally and linguistically appropriate services.
- System capabilities: The site uses electronic systems to collect data and execute specific tasks.
- Measure and improve performance: The site systematically monitors performance and carries out activities to improve clinical outcomes and patient experience.
CityMD plans to pursue NCQA Patient-Centered Connected Care Recognition for all 52 of its locations.
