Therap Introduces Outcome Tracking Tool for Measuring Quality of Supports
LTSS software includes tools for ensuring effectiveness of person centered supports
WATERBURY, Conn., Oct. 11, 2017 /PRNewswire/ -- Therap has introduced the Data Driven Outcome (DDO) module for tracking outcomes and for measuring the quality of services and supports provided to individuals. With the DDO option, Therap continues to expand upon tools that enhance the person centered documentation process.
Therap users, including caregivers and other service professionals, providing supports to individuals with developmental disabilities and other Long-Term Services and Supports (LTSS), can utilize this tool to create customized measurement of outcomes for individuals, assign scoring labels and list tasks to be completed.
The Data Driven Outcome feature can be used for aggregate reporting and is available as a part of Therap's Business Intelligence platform which produces graphical representation of agency data displayed within intuitive dashboards. Users are able to aggregate reports on outcomes and analyze the effectiveness of the supports provided to the individual. This data can be analyzed at the individual, program, provider, and oversight level.
Jason Laws, Business Development Consultant at Therap Services, states, "Data Driven Outcomes is a natural extension of Therap's existing person-centered planning tools as it allows agencies to not only assess outcomes at the individual level, but to expand that analysis across all their programs and services. This addition has tremendous potential for agencies who are looking to evaluate their performance in supporting individuals and facilitating outcomes at every level of their agency. In an environment where demonstration of outcomes for individuals is taking on greater and greater importance over standard compliance models, this tool will be invaluable as agencies look to assess their performance broadly."
The Outcome reporting feature works in unison with Therap's existing person centered support planning tools. Therap's person-centered Individual Support Plan module brings the full picture of supports and outcomes together, and allows support professionals and case managers to reference goals and the status of individual outcomes. Interconnected modules ensure that service data collected in Therap generates reports and drives the progress of outcomes. Support professionals, program managers and QIDPs can effectively create and customize an ISP program by using existing templates on Therap. Users are able to generate comprehensive reports that on the services provided to the Individuals to track service delivery and progress towards goals.
